Why can’t I have two outputs simultaneously from an Echo?

I want to connect my mother's Echo 2nd Gen to her hearing loop amplifier. I can do that by adding a Bluetooth receiver to one of the hearing loop amplifier's inputs. However, if I select the Bluetooth speaker option on the Echo, the internal speaker goes off. Why can't I have both?

Is there any other way of having the hearing loop and the loudspeaker at the same time? They are in the same room, so need to be synchronised. Does the 3rd gen Echo have this facility?

I considered getting a Bluetooth 5.0 transmitter with dual audio. I would need a separate Echo Input connected to it, for input; the original Echo would serve just as a Bluetooth loudspeaker on one channel, with the hearing loop amp on the other channel. Would that work? Even so, it seems like an awful lot of boxes!
